What to Bring to Your First Appointment

  • Your insurance card
  • List of prescriptions and/or over-the-counter medication, including dose and frequency
  • Information about your medical and surgical history
  • Recent x-rays or relevant records

Please arrive early to your first appointment to complete a patient history form.

Our Notice of Privacy Practices describes how medical information about you may be used and disclosed and
how you can get access to this information. It is given
to all new patients, and a copy is available upon request.

Hospital Affiliations

We are on medical staff at Comanche County Medical Center.

Insurance and Payment Information

We are providers for Medicare, Medicaid and most major insurance plans, including Blue Cross/Blue Shield, Aetna, Federated, First Health, HealthSmart, Humana, Superior, TML, TriCare, CBA, Definity Health, and UniCare. We provide insurance billing. All billing is done locally. You are responsible for charges that are not covered by your insurance.

Your insurance company may require a co-payment at the time of your appointment. When necessary, our staff will work with patients to set up a payment plan.

If you have questions regarding billing or which insurance plans we accept, please call us at 325-356-7530. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.
